Opening a new child care facility or making significant changes to an existing one?
As part of the child care licensing process, providers must complete a public health plan review before opening a new facility or making certain modifications.
PLAN REVIEW
Submit Form 5000 to begin the plan review process. Reviews may take up to 30 days, so be sure to plan ahead.
